1. Early Bird Registration Rate ($35) applies if paid in full by Thursday, December 15.

2. Back-to-School Registration Rate ($45) applies if paid in full by Thursday, January 5.

3. The Final Registration Rate ($55) applies to all payments made AFTER Thursday, January 5.  (Thu, Jan 12 is the final deadline to sign-up for V-Day #3.)

Velocity VETS Registration Form

WELCOME, 2011-2012 VELOCITY VET!
Before you get started on this form, there are two important things you need to know:

1. This form is rather lengthy, and there is no “save” feature. That means you need to make sure you have a good 20 minutes to complete the ENTIRE form in one sitting, and then hit “Submit” at the bottom of the form.

2. Please do NOT try to complete this form without your PARENT or Legal Guardian! Not only do you (as a minor) need their permission to join, but there is also a whole section below that your Parent/Guardian must complete on your behalf.

Once you have your Parent/Guardian with you and 20 minutes, you may begin!
