2. The Final Registration Rate ($40) applies to all payments made AFTER Wednesday, January 25.  (The final deadline to sign-up for the TA Mid-Year Workshop is in-person, at the event itself on Sat, January 28 at 3pm.)

WELCOME, 2011-2012 TEEN ADVISOR!
Before you get started on this form, there are two important things you need to know:

1. This form is rather lengthy, and there is no “save” feature. That means you need to make sure you have a good 20 minutes to complete the ENTIRE form in one sitting, and then hit “Submit” at the bottom of the form.

2. Please do NOT try to complete this form without your PARENT or Legal Guardian! Not only do you (as a minor) need their permission to join, but there is also a whole section below that your Parent/Guardian must complete on your behalf.

Once you have your Parent/Guardian with you and 20 minutes, you may begin!
