If you would rather download and print out a paper copy of your Registration Forms, simply click on the appropriate link below.  You will be redirected to a PDF file of the forms that you and your Parent/Guardian can print off and complete by hand.  Once completed & signed by your Parent/Guardian, mail your Registration Forms in to:

Online Registration is as easy as 1, 2, 3!

1. Register.  Click on the appropriate online registration button below to complete & submit your online Registration Form!

2. Pay.  After you submit your Registration Form, hop on over to the payment page where your Parent/Guardian can complete your registration payment online through PayPal.  They can use their credit card, debit card, checking or savings account.  Click on the appropriate link here to jump directly to your payment page:  Velocity  |  Vets

3. Release Forms.  Your final step will be to click on this Legal-Medical-Media Release Forms link and print it off for your Parent/Gaurdian to sign and send in!  That's it!  We can't wait to welcome you to the team!
